    In compliance with the provisions of the Family Education Rights and Privacy Act (12 U.S.C.: 1232G), I hereby give permission for a Staff Council Office employee to verify my grade point average and employment start date. I understand that the application materials will be held in the University Senate and Staff Council Office for one year and are then destroyed.
